Waiver One(Required)

I understand this is only an initial request and does not guarantee my restaurant night will be approved. If accepted I will be required to provide federal tax ID, W-9 and proof of nonprofit letter for the organization. I will not promote the restaurant night until an official flyer has been received with an approval email from Urbane Cafe.

Waiver Two(Required)

I understand orders that are placed online at ordernow.urbanecafe.com for “pickup”, “curbside” or “delivery” are eligible if the coupon code is entered at checkout. Third-party orders are not eligible and will not be counted towards the proceeds.
